Power of Attorney (POA):

  1. Miscellaneous form
  2. POA (Notarized from local foreign office)   to be attested by the Embassy, 
  3. Copy of passport along with supporting docs on whose name POA is transferred.
  4. Your original passport and self-attested copy of it,
  5. Two passport size Photographs  (One to be pasted on the form)
