2.      Indian Passport with one photocopy of the information pages.
3.      Two Photographs.  (One to be pasted on the form)
4.      Marriage Certificate (Apostille by Ministry of External Affairs) or Spouse name on the Indian passport

FEE: USD 40/- +  ICWF USD 02/- = USD 42.00

Bank charges - US$10.00 with all the services if the fee is paid through bank draft/banker’s cheque in favour of the Embassy of India, Paramaribo.
